Records Management Forms

Use the Records Officer Designation and Responsibilities (RM-25 Form) to designate a new, replace an existing, or add an additional Records Officer. Use the Records Technician Access Authorization (RM-26 Form) to designate a new, replace an existing, or add an additional Records Technician.
